The Gloucester Township Rabies Clinic is Saturday, October 18th from 9 am to 11 am in the municipal building parking lot, 1261 Chews Landing Road. To attend the rabies clinic, we ask that you complete the following form for each pet. Once you complete the form and click submit, a window will appear requesting your name and email address. Please provide a valid email address. This email address will be used by Adobe to send a confirmation email from Adobe Sign that you must respond to in order for the township to receive your participation form.
You will then receive a phone call from the township confirming your participation and an appointment time will be provided, so please provide a valid phone number.
